Attached reproducing unit test (ZF-11376_test.patch) and a potential fix (ZF-11376_fix.patch). This only covers the XHTML case...should this also be implemented for HTML case? Not that I think anyone in their right mind would (or should) give a HTML3/4 document an XML declaration :P
