Around line 290 in Zend/XmlRpc/Fault.php:

    // Build response XML
    $dom  = new DOMDocument('1.0', 'ISO-8859-1');

Should read:

    // Build response XML
    $dom  = new DOMDocument('1.0', $this->getEncoding());

Otherwise encoding of fault XML is always ISO-8859-1, overwriting default charset of UTF-8 and making calls to setEncoding() useless.
